Elmiron® is the brand name for Pentosan, which is sodium pentosan polysulfate sodium (PPS).
It is prescribed to treat a condition known as interstitial cystitis, better known as painful bladder syndrome or bladder pain syndrome.
Made by Janssen Pharmaceuticals, there is no generic on the market for the drug and patients often take Elmiron® continuously for years.
Approved by the U.S. Food and Drug Administration in 1995, Elmiron® is a popularly prescribed medication that has sales of more than $150 million a year.
While it has been on the market since 1995, there is no warning on the label of any visual changes or side effect conditions.
However, multiple studies were published in 2018 that document a litany of side effects–in particular, an irreversible visual condition linked to long-term use of the drug.
As a result of those reports over almost two decades, the drug’s warning label was updated in Canada in April 2020 to include the following information:
- Pigmentary maculopathy has been reported with chronic Elmiron® usage.
- Patients should have regular ophthalmic examinations for early detection of pigmentary maculopathy.
- If pigmentary maculopathy is confirmed, the patient should consult with their doctor to determine if Elmiron® treatment should be discontinued.
It wasn’t until June 2020 that changes were made to enhance warnings in the United States on the Elmiron® package insert and warning label.

The more severe side effects of Elmiron® use have been documented in the cases filed since the drug’s release.
These side effects all tend to center around a person’s loss of vision–some of the most common include:
- Blurred vision
- Halo vision
- Reduced night vision
- Unilateral or bilateral blindness
- Vision impairment or loss
Other severe symptoms include retinal pigment epithelium atrophy–a condition that causes the photoreceptors in the eye to die off or malfunction.
In recent years, medical researchers linked Elmiron® to the development of maculopathy, which is a progressive eye disorder that can result in vision loss in both eyes.
While maculopathy does not often result in complete blindness, it deteriorates the central vision, the loss of which can cause “legal blindness” of 20/200 vision or worse.
There is no cure for maculopathy, though intensive treatment can slow progression.
Medical professionals have linked maculopathy to drugs like hydroxychloroquine and corticosteroids in the past.
In 2018, research studies started documenting incurable vision loss in patients who took Elmiron® long-term.
